Spatial and temporal trends in public knowledge related to lupus in Canada: A Google Trends approach

Systemic lupus erythematosus (SLE, lupus) is a chronic autoimmune condition affecting 1 in 2000 Canadians, with disproportionate impacts on women and racialized groups. These disparities are further underscored by a lack of public education and awareness about SLE, leading to delays in diagnosis and critical care. The purpose of this research is to investigate public awareness and knowledge gaps related to SLE in Canada, and how this varies spatially and temporally. Using Google Trends data, relative search volumes (RSV) and associated topics and queries were collected from 2004-present. Peaks (>0.50) and troughs (<0.25) were explored by conducting additional searches. Top search terms were collected and analyzed thematically. Search volumes for “lupus” in Canada hit an all-time peak in Oct. 2015 (RSV=1.0), correlating with celebrity Selena Gomez’s diagnosis with SLE. Additional peaks were observed across Canada in Jul. 2009 (RSV=0.58), Aug. 2016 (RSV=0.66), Sept. 2016 (RSV=0.69), and Sept. 2017 (RSV=0.78), all of which correlated with milestones in the development of new drugs for SLE. There was a marked trough across all provinces in Nov. and Dec. of 2020 (RSV=0.28), perhaps reflecting that SLE-related concerns were overshadowed by the ongoing COVID-19 pandemic. Overall interest was highest in Newfoundland (RSV=1.0), New Brunswick (RSV=0.83) and Manitoba (RSV=0.80). Top related topics and queries varied spatially, but encompassed the following themes: causes of lupus, diagnosis, symptoms, medication, and treatment. These results will be shared and triangulated through a deliberative dialogue with knowledge users. This will set a foundation for targeted interventions to effectively reduce SLE-related health disparities and improve SLE-related quality of life.
