“You don’t expect a man to make good policies that affect women’s health”: Exploring barriers to gender transformative policymaking and programming in Ghana’s health and WaSH sectors

Despite global efforts to promote gender equality in health and water, sanitation, and hygiene (WaSH) services, particularly in Low- and Middle-Income Countries (LMICs), Ghana, continues to struggle. As a result, there has been growing advocacy for gender-transformative policymaking and programming. These gender transformative policies and programs seek to drive changes in socio-cultural norms and power structures and address the root causes of gender inequalities. This paper presents results from a case study in northern Ghana that explores the barriers and opportunities to gender transformative policymaking and programming in Ghana's health and WaSH sectors. Semi-structured interviews with stakeholders in three districts in the Upper West Region of Ghana were conducted from July to August 2024. Participants were purposely sampled from government agencies (n=11), civil service organizations (n=7), and community leaders (n=12). Interviews were digitally recorded and transcribed verbatim for subsequent thematic analysis. Participants revealed institutional and community-level barriers impeding the full potential of existing gender transformative policies and programs. Additionally, suggestions were made on how to improve existing WaSH and health-related gender policies. The National Health Insurance Policy, the National Community Health Planning and Services (CHPS) Policy, and others were revealed as some existing health policies with significant potential for bridging gender inequalities in Ghana. These findings will inform the development of gender-transformative policies and programs in Ghana's health and WaSH sectors by providing policymakers and other stakeholders with improved insights from this sample of those working at the front lines.
